Prior to scheduling an initial evaluation, the following steps must be completed.
1. Click on the “questionnaire” icon to complete and submit the required documentation.
2. After completing the questionnaires, click on “documents,” then “upload AND SHARE” to submit the front and back of both your insurance card and your picture ID.
YOU MUST CLICK ON “UPLOAD AND SHARE,” and select the team member “Juliana Pires.” This step cannot be completed with the web browser Safari. Please use Google Chrome.
3. Click the credit card icon located at the top of the page, to the right of the practice name (Birch Grove Mental Health) and to the left of the question mark. A box will open that will allow you to add your credit card to the system.
